Current Job Opportunities:

Case Manager needed for Home and Community Based Services Program.  This Program covers 14 counties in the Southwest Georgia Area.
 
Minimum Qualifications:

• Four year degree in Psychology, Sociology, Social Work or related field
• Two years of experience as a Case Manager 

Preference will be given to those who, in addition to the minimum qualification, have direct experience working with a Community Care Services provider, or who have direct experience as a Community Care Case Manager.

• We offer a benefits package, along with a five-day workweek (M-F). 

RN or LPN needed for Home and Community Based Services Program.  This program covers 14 counties in the Southwest Georgia Area.

Minimum Qualifications

• Current RN or LPN license in the state of Georgia
• Two years case management experience required

Preference will be given to those who in addition to meeting the minimum requirements have geriatric nursing experience and/or home health experience.
